Q: How would this do for a practice amp for a Bass?

This speaker has very poor bass response. It has kind of a high pass at 100 or 140Hz.

Q: Can I plug headphones right into this? Or do I need some sort of jack adapter?

The Orange Crush 20RT Amp has a standard 1/4" stereo headphone jack. Unless your headphones have a different connector, you won't need an adapter.

Q: Is this amp good for a bass guitar?
