Q: Brand name ez ancor rates these at 100 lbs. why are these rates at 85?

Are the rating both horizontal or vertical pull?? I dont work for these companies, but my experience is that all anchors have limitations. I dont know what you are trying to hang. But if its that heavy thru bolt, or lag bolt into solid wood stud, or span the studs with furring strips. I would not trust any hollow wall anchor suspending a $1000 dollar TV.

I would say yes if installed properly.

Q: Can screw be backed out a bit to fit into mounting holes or does it’ll screw need to be tight and flush to wall?

When you say “screw” I believe you are referring to the fastener rather than the screw that mounts onto the fastener. If so, the fastener is going to lose some of its gripping power if it’s not tight to the wall. If you’re referring to the actual screw that mounts onto the fastener, I think you’ll be ok, although I don’t know why you would want to back it out.

I believe my walls are plaster and lathe (old house built in 1919 or so) and I used it to hang a mirror of about the same weight.
