Q: Are the hex screwdriver bits held in the ratchet with a magnet?

Yes. I have both the 3/8 and the 1/4 ratchet and the hex bits are held in my magnet and its really good hold

Q: Does this have a "neutral" setting where it doesn't ratchet either direction?

I do not remember it having a neutral setting. I am pretty sure it does not.

Q: What is the outer diameter of the knob?

The knob is 2" across. It works very well for my needs.
