Yes it is designed to fit inside the rectal area. I haven't used it in this fashion, but I should imagine it's quite comfortable as there are no seams anywhere on the device, it's curved all the way around the insertable part as well, with no edges.

It can go inside if needed. In all honesty I don't remember how long it stays cold. After it would get warm I would wash and refreeze for another application. It worked well for me.

Can someone explain the 3 different applicators? I did not receive them....

There are no applicators, the description is misleading. They are referring to the three different shapes-- it can be used internally with the antenna shaped one, or you can use the large or small rounded ends for external hemmrhoids. Hope that helps.
