Is the fabric slippery? My husband tends to slide down while sitting in a sling in a wheelchair.

I have used this sling for a while on a patient and while in a wheel chair I have seen no sliding. I use a pad on the wheel chair which is non slip so that may be the reason why. I really like this sling and has saved me a lot of back trouble.

The description says size medium and up to 600 lbs.. the size chart shows medium as up to 225 lbs. What size is this one? I need the medium. Thanks!

do you have to buy extra straps?

No, as you can see in the diagram... all the straps are attached to the corners of the sling itself. I love mine, we use it to put my son in and out of his bathing chair in the shower. when he comes out of the shower, this sling does not hold water and it dries quickly....does not drip all over the place after wiping it with a towel. This is a great buy and sure makes life fun for my son and lots easier for me.
