Q: Are there lights for the bottom middle of foot? Doesn't look like it.

There are lights in the middle of the foot.

Q: When in pulse mode are you supposed to feel it?

I don't feel the pulses when I've used pulse mode.

Q: Can I use this if I have metal implants in my legs?

It does not use microwave or electromagnetic energy so it's pretty safe. If the implants are not close to the surface it should be fine. It does get warm and if the implants get hot it could be a problem. Different settings may help. My doctor said it was fine for me.
