Will it fit in a 3.75" I.D. cast iron pipe

It will fit. You will need to take a few measurements to make sure the I.D. of the pipe is round because 3.75" would be the smallest allowable I.D.

I have a 4 inch pvc pipe with old flange glued on outside, can i cut flush with finished floor and use this flange?

You may have to remove the old flange and then should install our compression flange about 1/4" above the floor height. Then add a new wax ring on top. Make sure the subfloor is in good enough condition to help support the flange and you should be fine.
