I just received this 1/4" ratchet and when I tried it out I noticed it makes a very noticeable clicking sound. Is this sound normal?

Hello yes this is normal it is battery powered so they make different noises than those that take air power go for it enjoy.

does it comes with adaptor chaging 1/4 to 3/8?

No. They make it in 1/4 inch and also a 3/8 inch version.

When this stalls, does it lock up so that you can turn the whole thing by hand and thus turn the bolt?

Yes, it will turn like a normal ratchet when it has exceeded it's torque limit.
