What is the diameter of straws? Wide enough for smoothies?

I don't know the diameter, but some things might have difficulty getting through the straw if they're hard or very big.

Is it too big for a 12 oz cup?

Technically no but it will stick out quite a bit. I'd look for something shorter.

Is this okay for hot liquids too?

Yes, that is what I use, because of having BIG mustache, while drinking my Coffee! DRINK UP, HOT or COLD!
