Does this have to be unscrewed from under the counter every time to fill it?

Question 2

My current dispenser drips several times after pumping, leaving a puddle of soap below. Has anyone had this problem with this one?

Only a guess, a possibility, even probability: The liquid soap is too thick, i.e., viscous.  When mixing a 16-ounce batch, at maximum use 8 ounce actual soap with 8-oz water. I'd suggest less soap than that, 7 oz with 9 oz water.  The nozzle possibly is on the verge of "clogging" with the too-thick soap, a quantity remains within the tube, and as time passes the portion within the tube slowly dribbles out.  Give it a try - thin the mix; never, ever use soap straight from the bottle. I'd also suggest a thorough flush out and cleaning of the entire mechanism; pump pressure possibly is being held back, builds up because of excessive thickness of the soap...then slowly forces more soap upward and out.

Question 3

Can someone who purchased this confirm if an18 ounce refillable bottle is included as stated on Moen's website?

"Exactly 18 ounces?" - I have no idea, but it's at least 16, which is what I fill the container with in terms of volume, a 50-50 soap/water mix; full strength dishwashing soap seems to overwhelm the pump.  Likewise, a few months use and valve-sticking has occurred, the internal spring possibly slightly weak.
