Where is this knife made?

As with all of the advertisers. You must pay attention to the "Word Salad" used in their descriptions. 
Many companies are based in one place, and use taglines touting American this and America that. Designed by this person or this place, labeled for that company, etc.

In the end, if you care where a product is made and who benefits from manufacturing, You need to research where the knife is actually made and by whom. It's not difficult to conduct a search online using the right keywords to arrive at a more informed answer. Many American knife manufactures produce some models in the states and some overseas. Many overseas models claim European manufacture but they also outsource to other countries and do not disclose the details.  Good luck.

Question 3

A reviewer has complained that the blade is very thin. How think is the blade? I am worried about it potentially snapping.

I have had a few of these and have broken one of the blades. It broke when I was driving a peice of heavy machinery after I was thrown up and I came back down on it. The blade broke inside the sheath and the knife stayed safely in place. I did sharpen the back side to make it double edged so that may have compromised it, but I may have been injured if it had not snapped. So overall I appreciate it's not a quarter inch thick.
I have carried these knives about every day for years and haven't found a better knife to carry for what I carry it for. I've had several knives all under $100 and none of them meet my needs as much as this one for a double sided knife. I keep it concealed so it isn't always in the most comfortable place, but I can dispatch it quickly and I have no doubt it would do enough devastating damage to stop a threat.
About the only issue I've had with him beyond this is after taking it out of the sheath possibly a thousand times, the retainage diminishes. Even at that point it still stays in better than some knives that are brand new. For me this takes years since I don't take it out of the sheath very often and for the price I don't mind replacing it. Then the old one becomes another one I practice with so I don't wear out the retainage surfaces.

Does this come with straps to boot carry?

Mine did not come with straps, just the sheath...I used paracord to attach it to my backpack
