After installing a rivet, the back compressed down but didn't mushroom (flare) out-- rivet just falls out of the hole. What am I doing wrong?

This tool requires a few pumps. Keep the tool attached to the rivet tail and keep pumping the handle untill the head is compressed and the tail self clips.
