Looking at the photos there is no line, how long a line can i use?, about 4mil diameter.

Well I am using 10mm rope on my ham radio tower this is at least 20mtrs, I would think 2mm wire or cord could be 30mtrs , for the price its areal bargin ,best regards Ray.

is it possible to adjust spool size so can wind more rope faster, e.g metal inserts parallel to axle? seem to be holes made for this but can't see....

The spool side with the gear attached has several holes but these are blind. The other side has no holes to match. I suppose you could drill through and then fit the modifications you suggest. This would leave about 15mm depth for rope until the end of the flanges.

does the handle turn completely ie full revolution or operate like a crank?

The handle turns a full 360, it's not like a ratchet winch.
