Does this come with the headset mics?

The option for 2 lapel mics shows 2 belt packs pictured with 2 clip on lav mics AND 2 headsets. So I'd say yes. I, however, purchased the 2 handhelds option so I can't say for sure.

Does this system require phantom power?

No it requires an xlr cable with a male and female end to plug one end into your mixer board and the other end into the wireless unit. You can also us a dual 1/4in cable as well if your mixer board receives that as well.
