Is it okay for a 5'11" person to do sit-ups?

Yes, my son is 6 feet 3 inches tall and it works great.

Can the height be adjusted?

The height of the bench itself can't be adjusted, but the back can. The back can be adjusted to make the bench flat. A pin can be pulled to fold it up to a more compact position as well.

Just got mine and set it up. The 2 bolts that go in the foot supports don't tighten. Blue washer in nuts prevents that. Is that intentional?

That's a nylon lock tight washer, you should be able to tighten the nut till you can't,
