Yes, they rotate very slowly at the same rate the Earth rotates. Or alternatively, there are two buttons that allow you to speed the rotation up a bit, both clockwise and anti-clockwise.

I've recently purchased this for our 4-year-old and can confirm that the images do rotate (four speeds available). It is reasonably quiet, you can hear a little bit of noise while it rotates - you can also opt for it to stand still. I think you can buy a separate adaptor, just a standard one, but I haven't done. It's still running on the batteries it came with and it's lasted really well. You set the images to rotate according to today's date and I like that function: you see exactly what your part of the sky looks like that time of year. It comes with discs for both Northern and Southern hemispheres which I also liked, in case your nephew wanted to see what it would look like on the other side of the globe! Hope this helps! I would recommend it!

There are two slide lenses that come with it. One has constellations markings and the other is the projection of a starry sky with no markings.

How big can the image be and remain in focus?

When I project it on the ceiling it's about 2-2.5 metres across. You can always manually focus the lens, so it could probably get bigger provided you have enough space (it needs at least a couple of metres between the projector and the surface you're projecting it on).
