Does it come with a hook included for hanging in the shower?

It doesn’t come with a hook. It does have a hole on one end to hang It if you have a hook or something to hang it on.

How often should you replace it?

It really depends on how much it's used. I've had mine almost 4 months now and it's still in good shape after daily use.

Do i have to clean or disinfect it

I rinse it off after every use and it seems to be holding up very well.

Any thoughts on making a long handled version of this for a back scrubber?

It's too soft to really be a scrubber, so handle unneeded.
