Is there a way to use top right button & bottom left together (& vise-versa) to make a new command for the Kensington Expert Mouse?

You must install the Trackball software, which starts every time you reboot. it's a free download from Kensington's web site. With that you can define a function when you click both buttons together, and also define what each button does on its own.

Does it have Windows 8 (specifically 8.1) support?

It works fine with my 8.1 x64 install

Are these easier on the wrists considering that the pictures show hands in the horizontal position, which is usually bad for your wrists.

I've been using a variety of trackballs (finger operated not thumb-balls) for months now and I can't go back to using a mouse. i've had literally zero wristproblems since switching for everything (even gaming) and whenever I'm forced to use a mouse even for a short time now I am quickly reminded of my arm / wrist aches again.
