How do you get the cover off to access the fixing points?

Put a small screwdriver blade to each side in turn and apply a small amount of pressure and it will ease off

So I fix things with screws and rawl plugs every day, but this seems to be a sealed unit, I.e. I can't separate the bracket to fix it to the wall.

The one I bought had top and bottom screw holes and snap caps and was easy to fit. Good luck.

You want the 100mm option for 4 inch good quality grills I have 4 of them in total
