Does a Q-tip fit in this pouch?

Yes, the dimension of the pouch is 5.3"(H) x 3.9"(L)

Can it hold a field note?

This think looks cool but it's just a weird size. Probably too small for field notes. I use if for a small flash light, bic lighter and a couple of cash bills. Probably better to find something a little bigger, not only to hold the field notes, but a pen as well.

Do handcuffs fit in it?

I didn't test the items you are referring to. But I think they will work together.
