It’s a standard hydraulic fluid fill cap. This caps fits a hole with a 2.25” OD.

Does this cap come with metal flange plate that screws to the tank?

NO IT DOES NOT. There is no mounting flange with the cap and chain. Although the picture shows there is one, the item name is correct. There is no flange with the Cap.
