It says that it is manufactured by Advance Pharmaceutical, also out of Holtsville NY. If they send some of their products to another country to get manufactured there I don't think there's any way of knowing.

Yes if you take them BEFORE you eat the dairy food. If it is very high in lactose, take more during the meal. I take several pills before the meal. Good product.
