Negative. This is designed specifically to have the child harnessed behind you.

Is it dot approved. will i get pulled over by law enforcement, if i use this?

Most states do not require children to have any kind of restraint for a motorcycle. Most do have a minimum age for them to be passengers. We have not been pulled over while using it last summer.

What waist size will this accommodate ( the adult) ?

I wear a 40 and it fits me, it also has room for a larger waist line

Is this DOT approved?

I'm not sure about dot approved. In Iowa, there isn't much for child safety laws on a motorcycle other than obviously a helmet. My 7 year old can ride along comfortably and is very secure going up to highway speeds.
