Elite Core RP1U-16D Rack Panel with 16 D-Series Punch-Outs, 1U

Holes lined up on mine, nice and hefty, solid metal, no defects with mine. Not many of these allow for 16 inputs and this is exactly what I needed for patching in and out of racks, plus I have some leftover slots I can fill with Midi and USB patch-throughs.

Sturdy, all-metal panel for standard D-style ports. This is probably the MOST number of D-type ports I can squeeze into a single 1U rack space. It's great to have the flexibility of 16 different ports but be sure to do the cabling in the back prior to loading up your adjacent rack devices (or expect to spend time with a flashlight and some cussing).

I did not have the problem of the offset holes as others have stated. Mine came perfect with no issues and works as intended

Pricey... but quality.
