How wide/tall is the square at the bottom? I want to get a customized plate with my cats name to attach but need to know measurments first.

3" high base is square approx.1 1/2" to 2" but you should wait to get the item and make sure.

Does it have an opening on the bottom so it can be used as an urn?

No there is no opening, I am very sorry for your loss.
