Will these layer well under medical scrubs?

YES! They are nice snug without being overly tight. As a mom, I appreciate fitted clothes that don't make me feel like I am 20 but don't make look like I am trying too hard. The material is not see through which I also appreciate.

5'0', 170 lbs, Large or XL?

I am 5’4” 180 and I bought large. It fits snug if u like looser shirts an xl would prob be better.

do these pill? I hate when shirts pill after a few washings

No pilling so far and I've had mine for a year. However, my first order of 2 was for a black and an anima print. The second order was for two gray shades. The second order had more stretchy fabric and fit kind of snug. I like the first order and was hoping for the same shirts. Both are o.k. but I liked the looser fit better.
