Is this recommended for achilles tendonitis?

I used the heel cups for plantar fasciitis on the advice of my podiatrist. They were fabulous! But I'd ask your podiatrist about whether or not they would help with achilles tendonitis.

Is it slim enough to fit into flat dress shoes?

Yes. I have worn them in flats and sneakers. Very comfortable, and you could probably even trim them to fit your shoes, if necessary.
