Is the compression pad flat or does it have a lump/bump in the middle of it?

It is flat if you use only the belt. I comes with a circular pad not attached you can put and then wrap around. My husband did not use the pad. It's a loose pad to use or not use. He just had hernia surgery on his stomach so he used it prior to the surgery.

Does this belt have a soft plush lining?

Does this have round or square ends? Square is best. Makes a huge difference in comfort. Round exposes velcro making it itch and grab clothing.

It has square ends and the connection is very secure. I have to caution you to order a size a little bigger than you think. I ordered according to my size and found it to be a bit too small. It still works but I had to really fight with it at first to get it to fit.
