Do these come packaged individually?

They are not packed individually, they are in separated plastic sections. You could cut the package where they would remain in a separate section.

Does this come unhinged while brushing?

Nope, I have not had it come unhinged while brushing at all.

The listing says the charcoal acts as a natural cleanser, does that mean you don't need toothpaste?

I don't know anything about charcoal but I use toothpaste and it works wonderfully. I never considered using charcoal.
