how long are these pen refills from tip to end? i need it to be a 4.5in. refill.

From end to end the refill measures 99mm or 3.897 inches. Apparently you may need a different brand. Dr. Grips are one of the best if you use a ball point for extended periods!

Does this refill work in the 4+1 multi pen?

Yes. I just bought and tried in my 4+1. Fits perfectly... a little tricky to install though.
