Would it handle a max of 30 amps @ 12 volts DC?

Not sure on 30 amps. I have 15 amps @120v and its holding up. Thinking it would depend on how long it is holding 30 amps. There's not enough contact area to hold 30 amps constant.

What is the maximum dc voltage? 13? Or 220V?

I'm running 120v through mine it is working just fine.

How do you wire this? Wiring diagram in the pictures isn't helpful.

I put hot 120V into 1 along with a jumper to 3. Then jumped 3 to 5 and 5 to 7, so all of the odd numbers had a hot. Then I wired my 4 speed fan, low to 2, medium low to 4, medium high to 6 and high to 8. Worked perfect for me.
