I don't think it'll hold a 12 Oz can. The circumference may work, but the second strap is likely too high, and cans would probably fall out. That is, if we're talking about 12 Oz soda cans and such.

Q: how does it hold on to the backback?

It holds on pretty well. weave it, like you would other MOLLE attachments, and it should hold fine.
